2: Who banned you? Autoban
3: Why did you get banned? I left game
4: Reason that forced you to break the rule? PC crashed
5: Explain to us your general vision about the situation. Not my fault: PC crashed. My friend ask to wait me but they dropped me. Game was obviously lost for my team, but I never appositely leave games (just check all my games: I have leave rate 1% for this reason).
