Especially this part:

Without this it will most likely be denied.
You should watch replay and post some exact situations here.

Since it's his 1st post it's a ragequit report. He lost a game, lost his cool and reported without loosing time to check if the guy really is a cheater.
Next time maybe he'll follow the rules.
