He decided that we are on neutrals and refused to play with team. Untill the end og the game, he were only on neutrals. Around 51:30 of game time we stood by fountain, when Nevermore alone were killing Chen. He need to make only 3 steps to at last save Chen, maybe even kill Never. But he didn't help. On purpose.

Aprox. around 60 min of game play (after his last respawn) slark went AFK. We were in good position for win the game. Both sides were up, only mid was down. I destroyed last melee rags, so we get mega creeps. I believe we were able to win if only slark plays. I was returning to base, rest of team were close to respawn. We only needed to delay sentinel's push and watch megacreeps winning the game.

Slark simply went AFK and we have lost.

I didn't left on my own. W3 crashed.

We were in game with my RL friend AlmirHunter. Together on skype. All was fine, we were slowly winning.
Right after crash I started to take some pictures of the screen. Then I wanted to save gproxy log, but Almir told me I was kicked by vote, so there is no reason to do that.

He was highest level in Alliance with Highest K/D CS .

So it means if I will be highest lvl in the game I can break rules, like going AFK, avoiding team fights, cursing family, national intolerance and swear Islam and all turks alike? Wow good to know.

4: Tiny allways knew where we are. Even if we run away or be in invis, he was allways be in the right place in the right time.
5: 55:21 GAME TIME. (Sorry. I don't know how to watch replay - I never needed it.)
I was pushing sentinel's 2nd tower, when tiny came (it was expectable, so OK for now). When he came, I used U to run away. I let my fire panda go back home and let (mainly) earth panda die. I had min. 3 escape routes but even with my MS tiny were able to catch me with 100% accuracy.
There were more situations like this but as I said. I don't know how to watch replay so I cannot tell report times.

All rags and base towers were up. We were in some troubles (game was far to be lost), but luci started to jungle and didnt care about team fights. Even when he were warned:

...and he stood by the fountain untill the end of the game. (Except of one KS attempt - fortunatelly he failed.)
I wish him (and all leaver) to burn in deepest hell and I hope he will not get his 30 virgins after death. We were able to win. Even without him we killed all 5 sentinels, only in 4 players.

He simply decided, that game is over. We were playing 3v5, but we lost because they were unable to get the point of "stay together." I had score 11:5 and in team fights I were able to do hell.

From the start of the game untill aprox. 20-25 mins.

Thrall was afk aprox. half of the game time. Not for long period, but very offten. If you check his level and items, when we were around lvl 16, he were 10. We wanted to kick him several times, which should warn him, but enemy didn't vote. So he remained in the game and ruined it.
