5: Specify at least 1 exact situation where user broke MH rule, with every detail explained and replay time when he used it.   (replay time, not game time please)

Without it, it will most likely denied.

The best move in the game was probably at [00:16:18] game time - broken hook on Gayacle...
Anyway, gg wp, epic mh report, I'm really flattered and all, however I wasn't hacking, the game was just way too easy...
