2) Moderator's nickname (as in subject).
4K
3) Why you are not happy with Moderator's decision / behavior.
He ban last a week ago for game ruining ONLY ME, but its so obvious other player intentionally came on mid and ruined the game.

Buy new control would be nice better(reduced 6hours)
