I think you can't get ban for using MH on custom game, I think.

Imo you can get ban if you have mh in custom DotA game.

Yes, u can get ban no matter what u play  (with mh), anyway mher on pudge warz, mher on dota... same ..

If you play custom game created by PD bot, and you use MH you can get ban. But if you use MH on custom game on some private bot, you won't get ban on PD.

About leaving in custom games by PD bots, you can leave and you won't get ban. And I don't know about flaming and afk-ing
