If the ban is from a game (!bl command) - no.
If the ban is from a moderator - you should have a warn.

This is moderator's decision.
Example:
A guy who lost connection (he didn't plug) asks for unban. The moderator will unban him, but won't add a warn, because the player didn't break any rules.
