He died alot , but so did all , team was outfarmed/outplayed.
Do you remember any times when he Intentionally died [ run into tower,into several players,etc].

Getting owned or playing bad isnt Game Ruin, did he rush into Enemy's or Enemy tower's , did he Intentionaly died , or he got "Raped" at his lane?
